  3. Complete the claim form with your contact information.

  5. For mail submission, print and sign the form and mail it so it is postmarked by March 11, 2026.
  6. For online submission, submit by 11:59 p.m. Hawaii-Aleutian Standard Time on March 11, 2026.

• Without documentation, claimants may receive up to $7 per eligible product for up to three products per household(maximum $21), subject to pro-rata adjustment if claims exceed available funds.  


With Proof (Increases Maximum Payout)

• If you have valid proof of purchase (receipt, order confirmation, or similar documentation), you may be eligible for a payment equal to the purchase price of the product excluding tax, subject to pro-rata adjustment.  


You may only choose one claim type (with proof or without proof) and may not submit both.
